READY TO REGISTER?

To register for a training offered through Prizms, please use the electronic submission form below.  Fill in the requested information and hit "SEND."  Trainings offered through Prizms will have a Reg.Code in the course listing.  You will need to complete separate submissions if you are registering for more than one training.  (Please note that trainings offered through Northwest AHEC will be registered through and paid to NW AHEC, not Prizms.  These offerings will not have a Reg.Code in the course listing.)  
 
If you prefer to submit your registration information by FAX or MAIL, you may print off the Training Registration Form found to the right.  Use separate forms for multiple training registrations.

YOU'RE ALMOST DONE, BUT NOT QUITE . . .

To finalize your registration and hold your space in a training, you will need to submit full payment.  It is recommended that you pay online by charge/debit card (VISA, MasterCard, American Express, Discover) with our secure PayPal Service using the "button" beside a specific training on the "Trainings and Workshops" page or also found under the PURCHASE ONLINE menu tab, or you can mail a check to Prizms or coordinate check/cash payment directly with Prizms.  (See CONTACT US page.)

If you need Prizms to enter your charge/debit information on your behalf, you may use the Charge Authorization Form below to FAX or MAIL your information/authorization to Prizms.

Refund Policy

Requests for registration refunds must be made in writing to Prizms by mail or email.  If a registrant cancels registration 14+ days before the beginning of a training, the full registration amount minus a $15.00 administrative fee will be refunded.  Between 7-13 days before a training, 50% of the registration amount will be refunded.  Registration fees are non-refundable less than 7 days from the beginning of a training.  Exceptions to this policy are solely at the discretion of the Director of Prizms.  Prizms reserves the right to cancel or postpone a training in certain situations, including, but not limited to, the following:  minimum registration numbers not met; or unexpected instructor unavailability (i.e., illness).  If Prizms cancels or postpones a training, current registrants would be offered placement in substantially equivalent trainings or would receive full refunds. If a refund is made, Prizms will make every effort to provide the refund within 30 days of the written request.
